βsum = −0.2201×(age/10−7.036) + 0.2467×(male−0.5642) −0.5567×(eGFR/5−7.222) + 0.4510×(ln[ACR]−5.137)
Risk = 1−S₀exp(βsum), where S₀ = 0.9832 (2-yr) and 0.9365 (5-yr) for North American cohorts. Validated for eGFR 15–59 mL/min/1.73 m²; interpret with caution outside this range.
